I just read some people are considering using only one item at a time. Just as a reminder: you'd also lose the awesome dual grapple bow swinging, so you'd have to alter the second tutorial.

Well, you'd have to switch to the 2nd grapple to do that now. It'd still be possible, just more annoying.

Couldn't you also change the grapple bow thing to be able to fire two shots?

Of course. But then you could ALWAYS do that. Now it only works if you actually have 2 grapples. I guess it would be only possible if there were 2 in the inventory, but that seems rather unintuitive to me. (Special controls for non-basic stuff, randomly placed)

No, I mean change the grapple bow so that one bow has two grapples in it.
